The cost for regular visit: $14.70 per session for Non Health Card Holders Home visit: $15.00 Concession Card Holders $9.60. Children/Adolescents (0-18 years):Free.
Referral sources: G'Ps, Self, Family members, Carers, Community Agencies or Health Care workers. Referral documentation required (from a referring agency): Victorian Statewide Referral Form (VSRF) or SCTT Confidential Cover Sheet, Consumer Information & Summary & Referral templates. All referrals (individuals or from a referring agency) must highlight any risks, allergies or alerts as applicable. Failure to provide the supporting referral documentation may result in delays in processing the referral or the referral being declined. Community based and / or centre based exercise groups are also available and Medical Clearance is required. If you require an exercise group please indicate this on the referral or speak to the Intake worker.
