CARDIAC SERVICES
We are a Nurse led management program for patients with Congestive Heart Failure and/or Cardiovascular Disease across the continuum from hospital to home.
HEART FAILURE
We provide collaborative inpatient education for acute presentations of CCF including self management strategies, symptoms recognition, discharge planning and medications.
Ongoing follow up in the QE2 district when required by telephone, outpatient clinics, and/or home visits to monitor heart failure status and enable optimisation of medication therapy.
Referral to slow stream exercise programs, other allied health/services and palliative care as appropriate.
CARDIAC REHABILITATION
Inpatient education and support for ACS, acute MI, and patients with risk factors for CVD including explanation of CVD, self management strategies, pending diagnostic and interventional procedures and discharge planning with medications.
Referral to appropriate Phase 2 cardiac rehabilitation and other Health Care Professional's when appropriate.
We are based on the 3rd floor and operate within business hours, Mon-Fri. Referral via medical officers are welcomed either by telephone, personal consultation or written form

• Smoking and alcohol history.
• significant symptoms, e.g. chest pain, breathlessness, syncope or palpitations
• exercise tolerance
• risk factors
• results of any recent cardiac investigations: (ECG, CXR, Echo, stress test)
• LFT, FBC, TFT, cholesterol, electrolytes
• details of any prior cardiac surgical/interventional procedures.
