CVPCP is integrating service coordination practices, processes, protocols and systems across the continuum of care by the:
* Development of multi-disciplinary care pathways;
* Collaboration; .
* Integration and service system alignment, including electronic services directories and e-Health capacity. The Service Coordination Continuum of Care model aims to detect risks, care priorities and needs early and to use a common, coordinated, collaborative approach.
