This may include education on general nutrition, heart disease, weight reduction, diabetes, eating disorders, fibre and modified fat diets. All dietary adice is planned individually with the client.

The Diabetes Educator provides information and support for people with diabetes such as:
• Explaining what diabetes is
• Assistance with the 'day to day' management of your diabetes
• Teaching blood glucose monitoring including: getting set up, what the results mean and what can affect the results
• Providing information and explanation on various types of medications and insulin
• Problem solving, to help the person take control of their diabetes
This service is available on Mondays, at the Beaufort and Skipton Medical Practices.
Fees
No fee will be charged if you have a referral from your GP, as part of your Diabetes GP Management Plan and Team Care Arrangement, or there is a $10 fee.
What should I bring along to my appointment?
It is helpful if you are able to bring a list of current medications, your blood glucose record book, blood glucose meter and equipment, to your initial appointment with the diabetes educator.

Post Acute Care is a time limited short term intervention designed to assist patients to recuperate following an acute or subacute hospital admission and to facilitate their independence, or transition to continuing care when required.
This care may include, for example, home help, personal care, meals support, respite care and home nursing.
The Case Manager will work closely with the client and/or carer and health care workers to ensure that the most appropriate Post Acute Care is provided to those clients with identified needs.
All clients referred and accepted into the program will have:

-- Post acute care plan prepared
-- Appropriate services negotiated
-- Care services monitoredThe care needs of the client will be reassessed at the end of the allocated time on the program and be discharged or referred to the appropriate care.

Aims
The program aims to achieve a smooth transition for those clients with more complex health needs, returning back into the community from an acute episode in a public hospital, either acute or sub acute facility in Victoria. This will be achieved through the provision of coordinated, appropriate and timely Post Acute Care Services.
Program objectives are:
-- Provide additional home-based services to assist individuals to recuperate after hospitalisation;
-- Improve continuity of care for patients following their discharge from hospital;
-- Work collaboratively with hospitals to provide support to patients as an alternative to inpatient admission from the emergency department, where appropriate; and
-- Assist hospitals and sub-acute services to maximise use of beds for patients who require hospital services.

A service which provides in-home nursing and treatment by nursing staff to assist people to remain living independently in the community. The District Nurse provides care in the home for long or short term illnesses. Anyone who has a health problem or well being problem that affects their normal daily lifestyle or their carers is eligible.
Care and treatment include: support care, rehabilitation, surgical dressings, medication, assistance with personal hygiene, stoma care, palliative care, adjustment after hospitalisation, prosthesis care, personal care alarms, specimen collection, assessment and support for carers, health information advice and referral.
