The Community Rehabilitation program provides a multidisciplinary rehabilitation service to enable clients who are disabled, frail, chronically ill or recovering from traumatic injury to achieve and retain optimal functional independence.
Features of the program include:
• Holistic assessments which include the client's mental and physical health, as well as their social, environmental and economic situation;
• Goal-oriented and time-limited rehabilitation plans;
• Periodic review of progress, with modification to rehabilitation plan as appropriate;
• Effective discharge planning, ensuring appropriate follow-up and referral to other services as required.
The service is managed by the Allied Health Ambulatory Care Coordinator and is staffed by occupational therapists, physiotherapists, social workers, speech pathologists, prosthetists, orthotists, allied health assistants, podiatrists, psychologists (including neuropsychology), exercise therapists and dietitians.
Clients may be treated on an individual basis or in groups. Some therapy programs have been devised for specific groups.

Pulmonary Rehabilitation is an 8 week program that offers a 1 x 2 hour education and exercise session per week plus an additional exercise session per week. A medical referral and interview are required prior to group inclusion.
The group is suitable for people with Chronic Pulmonary Disease (COPD) (emphysema and chronic bronchitis), bronchiectasis, pulmonary fibrosis and pre and post lung volume reduction surgery.
The program is not designed to meet the needs of those clients who require services during an acute exacerbation or individualised treatment from their chronic lung condition. Participants are required to be medically stable and able to participate in a group environment.
Current smokers are not excluded, however they must show a commitment to quit smoking.
The program is an exercise and education program involving physiotherapist, doctor, dietitian, occupational therapist, pharmacist and psychologist.
The Pulmonary Rehabilitation Program is located at the Peter Heinz Centre, Queen Elizabeth Centre, Ascot St South, Ballarat.

Central Highlands Coordinated Community Care provides a range of programs which include case management and brokerage funding for the purchase of services.
These include:
-- HACC funded Linkages Packages
The Linkages program manages the provision of care at home for people who are frail, aged or have disabilities and who, without this additional support, would be unable to remain at home with safety and confidence. Linkages provides case management and is able tp purchase a flexible range of services to meet people's individual requirements.
No ACAS assessment required.
-- Community Aged Care Packages (CACP)
Community Aged Care Packages are designed to provide low level care equivalent to that received in a hostel. They are planned and managed packages of community care services to help older people with complex care needs to remain living in their own homes.
The types of services may include: Bathing; laundry; meal preparation; dressing; transport; housework; temporary in-home respite; home maintenance; and social activities. The services are very flexible and can change as client needs change.
Aged Care Assessment required.
-- Extended Aged Care at Home (EACH) packages
Extended Aged Care at Home packages are designed to provide a high level of care equivalent to the care you would receive in a nursing home. Under an EACH package a number of services may be provided. These could include help with: nursing; preparing meals; bathing; dressing; transport; laundry; minor home modifications; and social activities.
Aged Care Assessment required.Any person can make a referral to Linkages. For some types of service, an assessment by the Aged Care Assessment Service (ACAS) will also be required. A referral must include a signed consent of the person being referred. Referral forms can be obtained by ringing Central Highlands Coordinated Community Care Linkages on 03 5333 5566.

The Cognitive, Dementia and Memory Service (CDAMS) provides specialist multidisciplinary referral, education and advisory service for people experiencing memory loss, confusion or thinking (cognitive) problems and for their carers, and families.
Symptoms may include: Short term memory loss; personality changes - frustration or anger; loss of ability to do everyday tasks, failure to recognise familiar people or objects; wandering; anxiety; mobility impairment; incontinence; pain or speech difficulties.
CDAMS provides:

Provides case management, education and counselling for patients and their families. The service is suitable for people aged 6 - 65 years who are experiencing difficulties returning to family, work and other roles following brain injury (including stroke).
People with mild ABI are contacted by the Clinic 2 - 3 weeks after the incident to screen for any ongoing issues. The ABI Clinic involvement may last from one initial screening phone call, to ongoing complex case management for more seriously brain injured clients.

The Weight Managment Program is for adults who have obesity (BMI above 30) as one of their main health problems. The program promotes sound, sustainable weight loss through healthy eating, exercise and trategies to maintain behaviour change. It involves individual screening with a dietician, psychologist and exercise therapist.
For people with a BMI between 27-37:
Program structure: A six week intensive group program, followed by regular review sessions over 12 months. the weekly intensive program consists of 1.5 hour education sessions and 1 hour of supervised exercise. Past medical history and medication list needed to be included with referral

Prosthetics and Orthotics provides assessment, prescription, fabrication, fitting, review and adjustment of prostheses and orthoses as well as ongoing advice and care. Facial, digital and breast prostheses are not supplied by this service.
The service is available to children and adults who:
• May benefit from orthoses (braces and splints) due to reduced function resulting from musculoskeletal problems, pain, neurological conditions or systematic diseases
• Require prostheses (artificial limbs) for functional and cosmetic applications.
Fees
Prostheses are generally funded by Ballarat Health Services, or if eligible, the Transport Accident Commission (TAC), Workcover Insurers (WKC), and the Department of Veterans' Affairs (DVA).
Orthotic clients will be required to pay for their consultation and their orthosis. Some may be eligible for assistance via TAC, WKC, DVA, BHS, or the Aids and Equipment Program (A&EP).

Post Acute Care is a time limited short ter intervention designed to assist patients to recuperate following an acute or subacute hospital admission and to facilitate their independence, or transition to continuing care when required.
This care may include, for example, home help, personal care, meals support, respite care and home nursing.
The Case Manager will work closely with the client and/or carer and health care workers to ensure that the most appropriate Post Acute Care is provided to those clients with identified needs.
All clients referred and accepted into the program will have:

-- Post acute care plan prepared.
-- Appropriate services negotiated.
-- Care services monitored.
The care needs of the client will be reassessed at the end of the allocated time on the program and be discharged or referred to the appropriate care.

Eligibility
To receive additional post acute care services through the PAC program, clients must:
-- Have been a patient of a public hospital either acute or subacute in Victoria.
-- Be assessed as requiring additional services to assist with the recuperation or transition to continuing care following an acute care or subacute care episode.
-- Agree to the forwarding of individual client data to the Department for the purposes of program monitoring and evaluation.
Aims
The program aims to achieve a smooth transition for those clients with more complex health needs, returning back into the community from an acute episode in either acute or sub acute facility in Victoria. This will be achieved through the provision of coordinated, appropriate and timely Post Acute Care service.
Program Objectives:
-- Provide additional home based services to assist individuals to recuperate after hospitalisation;
-- Improve continuity of care for patients following their discharge from hospital;
-- Work collaboratively with hospital to provide support to patients as an alternative to inpatient admission from the emergency department, where appropriate; and
-- Assist hospitals and subacute services to maximise use of beds for patients who require hospital services.

• Have chronic conditions, such as obesity, back pain, arthritis, lymphoedema, multiple sclerosis, balance difficulties.
• Have specific medical conditions e.g. diabetes, cardiovascular disease.
• Have undergone treatment following an accident, injury or surgery.
• Are of retirement age and want to maintain or improve current physical fitness and general health.
Exercise Therapy provides physical activity assessment, program design, development, monitoring and review for gym, water, land and home based exercise programs. The program is not suitable for people who are medically unstable or are less than 6 weeks following surgery.
