Home Care Services Ballarat provide in home support to frail aged people and people with disabilities who wish to remain in their own homes but require assistance to maintain their independence. Support of Carers is a vital component of Home Care Services. Home Care Services Ballarat are committed to providing flexible high quality services which appropriately meet the needs of eligible clients and their Carers.
Food Services
Food Services are a part of the integrated home care services provided by the City of Ballarat and include:

-- Skill development in planning and preparing mealsThe Delivered Meal Service, available Monday to Friday, provides a variety of meal choices delivered to the home.
Fees
Fees are charged for all services, usually based on a sliding scale according to income. No one will be denied a necessary service because of an inability to pay. Council usually sets the fees schedule annually.
Charges for the service are assessed according to the income of a household. For pensioners there is a subsidised flat rate.

We offer a variety of activities such as:

The Sharps Safe container exchange program provides containers in public places for the safe disposal of syringes. When full, the containers are collected and the contents safely disposed of.
Ballarat City Council also provides a free sharps disposal service for insulin dependent persons. Sharps must be stored in glass or heavy plastic container and delivered to the Environmental Health Section for safe disposal.

Assessment and Care Management Workers are able to provide information on Council and other services in Ballarat for older people and people with disabilities. They are responsible for assessment of individual needs when people are referred for help and will arrange an appointment to visit the person at home.
Assessment staff can also assist with a range of care option available.
Assessment and Care Management Workers are also responsible for arranging a variation to the agreed care requirements.
Services available through the Home & Community Care program include Home Care, Personal Care, Respite Care, Home Maintenance and Food Services.

A Living at Home Assessment is a person-centred assessment of the person and their carer's needs, which leads to a care plan and individualised service responses. The assessment and care planning process sits within the service coordination framework and takes an ASM approach to:
• building on the person's and carer's autonomy, by offering choices and focusing on individual strengths and interests
• encouraging & supporting people to regain skills and capacities wherever possible
• strengthening social, community and family connections
A Living at Home Assessment is a broad, holistic needs based assessment and can include: a service specific assessment and care plan for the range of HACC services delivered by the assessing organisation; a referral action plan to both HACC and non-HACC services that cannot be met by the assessing organisation.
A Living at Home Assessment is a funded HACC activity delivered by designated HACC Assessment Services.
A SCTT referral is required from service providers – the referral should contain the following:
• Confidential referral cover sheet – clearly outlining the reason for referral
• Consumer information profile
• Summary and referral information profile
• Consumer consent to share information profile
• A copy of the Grampians Region Service Provider Home Safety Checklist (or your equivalent agency safety checklist)
• Any other templates considered appropriate/relevant
