Additional Information: Following a Living at Home Assessment (LAHA) and if a person is eligible for Home and Community Care (HACC) services, Delivered Meals may be provided if there is a need. The service aims to assist people who are nutritionally at risk or who cannot prepare food by providing a balanced hot, chilled or frozen meal to meet nutritional requirements. The number of meals provided to a person is determined at the time of assessment. A standard fee applies for this service regardless of income.

Additional Information: Community Aged Care Packages (CACPs) are individually tailored packages of care coordinated by a Case Manager provided to people 65 years or over with more complex needs. A broad range of services can be provided within a package that is personalised and flexible in order to meet individual goals and needs as they change over time. In order to receive this service a person must be assessed as eligible by an Aged Care Assessment Service (ACAS). Fees apply for this service and are set according to income.

The Healthy Ageing team provides a range of services to help older people, people with a disability and their carers, to maintain a healthy, active and independent lifestyle in their own home and within our community and to prevent inappropriate admission to long-term residential care.
The Healthy Ageing team progressively develops and implements a range of programs, activities and events targeting a diverse resident and visiting population of older people and people with disabilities. The Healthy Ageing team liaises with, supports, and provides information and advocacy to a diverse range of groups.

The service aims to assist carers to manage their caring role by providing appropriate regular assistance to the person they care for to allow them to have a break. The service may be provided in the home or in the community. Respite Care services may include a component of Personal Care dependent on the individual situation and circumstances. Respite Care is not provided on a one-off or ad hoc basis. Fees apply for this service and are set according to income.

The service aims to assist people who may be socially isolated to maintain and enhance their social activities and interaction through the provision of social support, activity-based and recreational programs. The service seeks to develop and/or maintain a person's skills, abilities or interests wherever possible. PAG services may also be provided to assist a carer to manage their caring responsibilities by providing them with a break from their caring role. Fees apply for this service and are set according to income.

Additional Information: When a person meets Home and Community Care (HACC) program eligibility criteria an appointment is made with an Assessment Officer for a Living at Home Assessment (LAHA). A person is visited at home and information is obtained about their abilities, needs and goals. If required a Care Plan is jointly developed with the person outlining any services to be provided and referrals to be made to achieve the goals of the plan. Information on Council and other community services available in the municipality is also provided. No fees apply for this service.

Additional Information: A subsidy for the provision of meals to community groups and organisations supporting seniors is provided on an annual basis. Community Meal Subsidies are available for seniors' organisations that meet and share a meal on a regular basis. Priority is given to groups and organisations that provide a healthy, nutritious and affordable meal option, offer a culturally appropriate meal if providing a meal for culturally and linguistically diverse seniors, and provide a range of activities or services that enhance senior's social opportunities and support an active and healthy lifestyle.
