HARP has a care coordination team with expertise in respiratory, cardiac and diabetes. The service includes access to nursing, occupational therapy, physiotherapy and other health disciplines based on individual assessed need.
We work closely with other members of the multi-disciplinary health care team, including GPs and facilitate onward referrals as necessary.

A qualified Occupational Therapist will assess the client and develop a personalised and comprehensive program to suit the client's needs.

What services are available?
Home Care - assistance with domestic cleaning
Personal Care assistance with personal hygiene.
To receive this service the District Nurse will carry out a personal care assessment
Personal Assistance - assistance with shopping, attending medical or health related appointments
Respite Care provision of a qualified worker to relieve a carer for short periods
Transport assistance with transport to medical or health related appointments
Nursing Care visits from qualified registered nurses to assist with wound care, medication and other nursing-related care.
To receive this service you will need a nursing assessment. This usually takes place prior to leaving hospital
Food Service in exceptional cases, the Care Coordinator may explore the option of providing meals.
Allied Health Services in some cases, the program can arrange for allied health services, such as physiotherapy or occupational therapy. This is a limited service.
How do I arrange Post Acute Care?
A program coordinator will visit you in Hospital, to talk to you about your needs and confirm that you are eligible for the program. After this assessment, the program coordinator will arrange for the services you need. A program coordinator will also contact you once you are home to discuss how you are going.

Hydrotherapy is a physiotherapy treatment carried out in water. It is conducted and supervised by a physiotherapist. The aim of Hydrotherapy is the rehabilitation of neurological, musculoskeletal and / or cardiovascular function of the individual.
The Hydrotherapy Program is held weekly at the Seymour Sports and Aquatic Centre.

Pre-Admission Clinic is run on an appointment system by SH Nursing Staff to prepare you for your surgery or procedure by:
carrying out a health assessment to determine your fitness for the planned procedure;
providing you with information about the procedure and answering of any questions you may have, and
planning additional services that may be required on discharge to meet your physical, social or psychological care needs
What will happen at the Pre-Admission Clinic?
The Nurse will explain what is involved with your hospital stay, your recovery, length of stay in hospital and going home from hospital
Your blood pressure will be checked, your weight recorded and you will be asked to provide details about your health and medical history
If further tests (i.e. heart trace - ECG, blood tests) are required, the Nurse will arrange these.
Following this assessment, the Pre-Admission Nurse may also request that you visit your Anaesthetist before surgery.
The Anaesthetist will assess your fitness for surgery and anaesthesia, recommend a type of anaesthesia to suit your needs and discuss pain relief options.
For Orthopaedic Surgery, you may also be seen at the Pre-Admission Clinic by the Physiotherapist and / or Occupational Therapist who will assess your physical needs and discuss exercise, recovery and going home plan.
What do I need to bring to the Pre-admission clinic?
Any medications (in their original packaging if available) or puffers currently being used (or a detailed list of amounts and times taken) including herbal and over the counter products purchased from health food shops, supermarkets or pharmacies.
Pacemaker - type and manufacturer details (if you have one)
Your blood group card (if you have one)
Any medical letters, x-rays, or recent blood test results
A list of questions regarding your surgery and/or recovery
You may wish to have a support person (family or friend) come with you, particularly if you have hearing difficulties or a disability that makes you dependant on others for assistance

Seymour Health provides a qualified Diabetes Education Program by a Registered Nurse who sees clients by appointment. Clinics are conducted at the Ambulatory Care Centre opposite the Hospital. Seymour Health does not have on-site doctors available at all times. Local GPs provide emergency and after hour care on an on-call basis.
The Nurse Practitioner or in-charge Nurse will assess the patient in the Urgent Care Centre to decide what level of care he or she might require. The provision of advice, support and care will be based on this assessment. This may range from non-urgent care to urgent care required within 10 minutes. The Nurse in charge will consult with the GP on call and medical care will be provided as required.
Patients who don't require urgent care may be referred to their GP the next working day.
The doctor on-call will be contacted if necessary and will come from their clinic or home to provide the required medical care. The attending GP will charge an attendance fee for this service.
If your condition requires more specialist investigation or care than can be provided at Seymour Health, transfer by ambulance may be required.
The patient is responsible for the cost of the ambulance transport.

The Cardiac Rehabilitation Program is designed to give clients and their family/friends the knowledge and confidence for healthy living through a 6-week program consisting of education, support and supervised exercise.
The program is aimed at both disease prevention and cardiac rehabilitation -therefore, anyone is welcome to participate.
Most people participating in the program fall into one of two categories:
people who have had heart disease or heart surgery, or
those who have been identified with risk factors leading to heart disease, such as high cholesterol, high blood pressure, overweight, lack of fitness or diabetes.

The Planned Activity Group provides a social activities program for eligible frail, aged and younger people with disabilities. Planned Activity Groups aim to meet the needs and interests of older people in a manner that promotes participation and enjoyment.
We are located in the Community Services building at the rear of the hospital.

The Renal Dialysis Unit at Seymour Health is a satellite unit under the Royal Melbourne Hospital. If emergency dialysis care is needed out of hours, you will require transport to the parent hospital.
If you feel unwell outside our hours of operations we strongly recommend that you contact your doctor or the Royal Melbourne Hospital. Avoid waiting until your next scheduled dialysis session, as delays may be detrimental to your health.

Admission to the Renal Dialysis Unit is arranged by the Coordinator of the Royal Melbourne Hospital Dialysis Service in Consultation with the nursing staff of the Seymour Health unit.
Holiday patients are accepted only if space is available.
