My Aged Care – Upload Documents



My Aged Care helps people navigate the aged care system, find information and connect with aged care services. It was established in July 2013 and is funded by the Department of Health.

Whether you’re looking for information or need an assessment and referral to a service provider, My Aged Care is your entry point.

My Aged Care's national phone line is managed by Healthdirect Australia.

During the COVID-19 restrictions many Australians have difficulties leaving their home to send documentation to My Aged Care. As an alternative, you can upload your documents via a secure, online form hosted by Healthdirect Australia on behalf of the Department of Health.
