GPs can download the templates directly into their software. Templates are currently available for Medical Director and Best Practice using Medical Objects or Healthlink. Templates for other software packages are expected to be available soon. The template is updated quarterly and is available at http://www.gpqld.com.au/eHealth/eReferrals 
If other templates are utilised, please ensure all relevant details are completed. Required information includes, Patient Demographics and contact person Medicare information (including expiry date) Reason for referral Specialist / Specialty referred to Relevant Clinical/Social & Family history Relevant tests/Investigation results Referring Practitioner and/or Usual GP (if different)

Our service is free. You don't have to have a Medicare card, but bring it if you have one. As a specialist sexual health clinic, we can provide you with a range of care:
Testing and treatment of sexually transmitted infections, HIV & hepatitis, Condoms/lube, Needle and syringe packs, Contraception, emergency contraception & pregnancy options, Post Exposure Prophylaxis (PEP), Ask us your sexuality questions (sexual problems, straight, lesbian, gay, bisexual, transgender or uncertain), Transgender care, Sexual health counselling and information, Indigenous Health Worker team, Sexual health and contact tracing information and resources.

Health Supplies
