annecto - David House

The annecto day service called Learning and Lifestyle provides planned support to people with a disability over 18 years of age on a daily basis both at Yarraville and in the community. There are a range of programs that are offered as part of this program including but not limited to educational based programs, art programs and recreational programs.

MacKillop Family Services - Community Programs

MacKillop Family Services exists to support, to foster hope and to promote justice for children, young people and their families, particularly those who experience distress, disadvantage and abuse.

This program assists people with complex care needs to live independently in the community by providing individually tailored packages of care. The program assists frail older people, younger people living with moderate to severe disability and carers with the aim to support them live at home and prevent premature or inappropriate admission to residential care. This program is supported by funding from the Commonwealth and Victorian Governments under the HACC Program.

Care Connect - Moonee Ponds

We are a major in-home care provider that supports people to live independent lives at home and in the community. We provide information, advice and guidance to assist people navigate and access support services they need to live the life they want. Our staff specialise in coordinating services across children, young people, adults and older adults. We value the diversity and provide services to people from culturally and linguistically diverse backgrounds, Aboriginal and Torres Strait Islander communities, people who are lesbian, gay, bisexual, transgender or intersex, people who are financially and/or socially disadvantaged and veterans.

Initial Contact: Duty Desk - 03 9243 8806  : To maintain and enhance an individual's health and well-being through the provision of a nutritious meal, to support the consumer's ability to live independently at home and in the community.  As this service provides regular contact it also plays an important role in monitoring clients' well-being and safety.

MOONEE VALLEY CITY COUNCIL

 Moonee Valley City Council provides a range of services and community facilities. Council also assists other agencies, community groups and individuals to provide other services through community grants, support and facilitation.

Services including attendant care, occupational & speech therapy, physiotherapy, training and consultancy in disability.

Services including: supported independent living, support coordination, independent living skills, tenancy supports, NDIS, community support and transition to work.

Lifestyle Solutions

Merri Health - Interchange North West (Pascoe Vale)

Provides recreation and respite opportunities for children, young people (6-18 years old) with disabilities and their families. Families with children and young adults with disabilities who live in Hume or Moreland. There is a fee of $15-20 per recreation group event plus additional fees for camps.

Melbourne City Mission - Disability Services Division - Thornbury

Services include Accomodation; Adult day care; Adult training centre; Case Management; Disability; Aged care; Residential care; Information/advice/referral; Therapy/counselling, Respite Care, Outreach Servcies, Community Visitors Scheme, Community Friends Program, Affirming Families, Siblings Program, Case Management Action Group, Parenting in Partnership Program, Acquired Brain Injury Services

DASSI

DASSI offers support services including direct care staff to people who have any form of a disability or condition that prevents them from being active in the community. Support includes personal care, home care, community access, social support, respite care, rehabilitation and specialist care.
