We support people in the community who have a functional disability, such as frail older people, younger people with disabilities and carers of the above. Services are chosen through a process of consulation with case managers, families and carers. Access to programs depends on eligibility criteria. Services include Community Aged Care Packages, Veterans' HomeCare, and National Respite for Carers.
