Following a Living at Home Assessment (LAHA) and if a person is eligible for Home and Community Care (HACC) services, Delivered Meals may be provided if there is a need.  The service aims to assist people who are nutritionally at risk or who cannot prepare food by providing a balanced hot, chilled or frozen meal to meet nutritional requirements.  The number of meals provided to a person is determined at the time of assessment.  A standard fee applies for this service regardless of income.

Ozanam Community Centre

 Ozanam Community Centre is based in North Melbourne and is a state wide service that provides support to men and women who are at risk of homelessness, homeless and/or marginalised. 
 
Working with a strong team approach we strive to provide a high level of best practice, innovative, respectful, thorough, mindful and flexible service delivery. We aim to provide a consistent, safe and comfortable environment for all. For some of the most marginalised and isolated clients who are unable to access traditional treatment services we provide basic needs and community on an on-going basis. We facilitate engagement with a wide range of services aiming to generate stable and sustainable living, autonomy, well-being and community connectedness.
 
Although it is a relatively small site Ozanam Community Centre offers breakfast and lunch meal programs, showers/laundry facilities, Housing Information and Referral, an Alcohol and Other Drugs clinician, a Client-volunteer Program, women’s programs, Community Aged Care Packages, Client Consultation Coordination, generalist support, information and referral as well as recreation and educational activities. In conjunction with allied health services it also provides on site doctors, a dentist, RDNS and optometry. Through government concurrent funding and sector partnerships we have approximately 20 permanent and visiting services available to clients. ww.a

It provides a range of services and support to improve health and wellbeing, if you are experiencing problems with drug and alcohol use. These services include a primary health clinic, access and referral to other support services, therapeutic and skill building programs, supervised recovery spaces and basic assistance including food packs, showers and washing machines.
