Hospital patients are seen by a consultant Geriatrician on request of relatives, outside agencies or the staff of the referring hospital unit. This service is offered at each campus.
ACCS assists with care and discharge planning for aged people.  Advice is offered about all aspects of the care of the aged such as acute medical diagnosis and treatment, nursing management, residential care - respite and long term, access to sub-acute care (geriatric rehabilitation and assessment services), community based services and care packages.  Direct links with the regional ACAS can provide further information about the patient's home situation.
