Kelaston

Nestled amongst shaded courtyards and gardens, our tranquil residence is located close to beautiful Lake Wendouree in Ballarat.
Kelaston offers 60 places, all of which are private single rooms with an ensuite bathroom. All rooms feature individually controlled heating and ceiling fans, and every room has direct access to shaded gardens and courtyards, providing a serene atmosphere in which to relax and enjoy the peace and quiet.


Talbot Place Aged Care Facility

Talbot Place is a modern, spacious 30 bed aged care facility located entrally on the Queen Elizabeth Centre site
in Dana St. Ballarat.
The facility is divided into two wings which are a mirror image of each other and have their own kitchen, lounge and activities area. The facility is open and spacious with skylights throughout providing an abundance of natural light. The lounges have flat screen TV and access to DVD players for resident use.
There are a combination of 14 single rooms with their own en-suites and eight double rooms with shared ensuites.
Each room has a clear view of the well-kept gardens and lots of natural light. Residents are encouraged
to personalise their space with soft furnishings, photographs and mementos. All rooms have access to
telephone lines and television antenna.
All residents' beds are electric and height adjustable. Sensor mats are provided as a safety measure for any
residents who are assessed as at risk. Each bedside area, bathroom and loung

Ballarat Health Services - Central Intake

Central Intake is an access and referral management service providing consumers; carers and patients with a single point of entry to those programs within the scope of Community Programs as well as facilitating navigation of the broader community service system.  Programs: Post Acute Care; Hospital Admission Risk Program; Grampians Regional Continence Service; Cognitive, Dementia and Memory Service; Allied Health; Rehab in the Home; Grampians Regional Palliative Care Team; Planned Activity Groups; Linkages; Aged Care Assessment Service and Carer Respite and Support Services. Central Intake is an access and referral management service providing consumers/carers/patients with a single point of entry to those programs in the scope of Community programs as well as facilitating navigation of the broader community services system

60 Bed Low Level Residential Aged Care Home. Facility consists of 3 houses, one house designed to cater for Dementia Specific Clients. All rooms are single with own ensuite. 

Interested or prospective clients should contact the Grampians Aged Care Assessment Service at 102 Ascot Street South, Ballarat, Ph: 03 5320 3740.  This service is responsible for assessing clients and providing approval for entry into aged care residential facilities.

Carer Respite and Support Services

Information and support for carer's and care coordination and linking to carer's respite services. The Carer Respite Centre provides a single contact point for carers to access respite care and information on other available resources to support them in their caring role. A Carer is a person who is looking after someone who is frail, aged, has a disability, chronic illness or mental illness, and the Carer can be family, close friend or neighbours. Respite care enables Carers to have a break from their caring role to allow them time to attend appointments, go shopping, social engagements or just some 'time out'. Care is available as short-term, occasional or emergency respite care.
