Hospital to Home: Short-term (4-6 weeks) additional support services (domestic assistance, personal care, home maintenance, flexible respite) for people who are frail aged or have dementia. This could be following discharge from a private hospital, or a short term extension of post-acute care. If the person requires longer term support (beyond the 4 – 6 week period), this support would be via a longer term Commonwealth Home Support provider.
