Transition care is a short-term, goal-oriented, time-limited and therapy-focused based program delivered in your home. It helps you complete your restorative process, optimise your functional capacity, and finalise and access your longer term care arrangements after a hospital stay. We provide a package of services that includes low intensity therapy such as physiotherapy and occupational therapy, social work, nursing support or personal care based on your needs.
