Home Based Care Foster Care provides a safe, nurturing home for children who are unable to live with their own family. Fully trained foster carers provide short, medium or long-term periods of care and are supported by a foster care worker. There are many different Foster Care options including Therapeutic (Circle Program) respite, short term, long term, permanent, and emergency.
This program includes foster care case managers who support the placement and case plan for children within the Home Based Care system through:
• Direct Support for children in care to reach their goals through the looking after children framework, e.g. Education, identity, self-care and all other areas of a child's life.
• Support for Foster Carers and their family's needs to support the child in care e.g. Care team establishment, reimbursement support and management, regular supportive home visits to carers, strategies to support any challenges.
• Case Management for all foster care households. This will include working through any statutory documentation for the child, advocacy, support, supervision and holistic understanding of the child's and carers needs.
Eligibility: Children up to 18 years old.
