Outreach support to people sleeping rough or homeless, Street To Home helps people access stable accommodation, health, social and welfare needs.: EntryProcedures For people who are homeless and sleeping rough in the Adelaide metropolitan area. If a person is eligible for our support the Duty Worker will ask about the situation to gauge what assistance is required. A face-to-face assessment is then arranged, and if required, a worker may be appointed to provide case management support.
