Short -Term Case Management (STCM) is a twelve week program provided to clients with complex needs who require support to achieve their goals.
A SupportFirst Advisor works in partnership with individuals to coordinate a range of services to ensure needs are met. They also offer advice, advocacy, referral to allied health professionals and assist in the development of support plans.
