A Case Manager provides a single point of contact for the person and their family to deal with the service system. Case managers work one on one with clients, families and carers to identify needed services and supports for the person. Who can we help? The target group is people with a disability, chronic illness or age related frailty who: - have multiple needs affecting their independence and safety - want to remain living at home in their local community for as long as possible - have a carer who needs support in order to maintain their caring role Acquired Brain Injury Case Management assists people with a moderate level acquired brain injury to remain living at home. People aged 18 years and older who have: - a diagnosed ABI, and - who are non-compensable, and - who require moderate assistance to maintain independent living. How do I access the service?
