The AccessCare Southern Short Term Case Management Program provides 3-6 months of case management for people who need additional support to understand and engage with the services and supports available to them. The Case Manager visits the person to assess, plan and implement a range of services and supports to meet their individual needs. The Case Manager understands the full range of services and options available. They will form a partnership with you to respectfully share their knowledge to help you to make the best decisions regarding your care. Where longer term case management is required the person is referred to a package of care e.g. Home Care Package or Linkages Package. Anyone can make a referral for Short Term Case Management by telephone, in writing or in person. Our Intake Worker can assist you with the referral process.
