Post Acute Care is a time limited short term intervention designed to assist patients to recuperate following an acute or subacute hospital admission and to facilitate their independence, or transition to continuing care when required.
This care may include, for example, home help, personal care, respite care and home nursing.
The Care Coordinator will work closely with the client and/or carer and health care workers to ensure that the most appropriate Post Acute Care is provided to those clients with identified needs.
All clients referred and accepted into the program will have:
-- Individual assessment
-- Needs identification
-- Post acute care plan prepared
-- Appropriate services negotiated
-- Care services monitoredThe care needs of the client will be reassessed at the end of the allocated time on the program and be either discharged or referred to the appropriate care.

Aims
The program aims to achieve a smooth transition for those clients with more complex health needs, returning back into the community from an acute episode in a public hospital, either acute or sub acute facility in Victoria. This will be achieved through the provision of coordinated, appropriate and timely Post Acute Care Services.
Program objectives are:
-- Provide additional home-based services to assist individuals to recuperate after hospitalisation;
-- Improve continuity of care for patients following their discharge from hospital;
-- Work collaboratively with hospitals to provide support to patients as an alternative to inpatient admission from the emergency department, where appropriate; and
-- Assist hospitals and sub-acute services to maximise use of beds for patients who require hospital services.
