Community Based Rehabilitation (CBR) provides a rehabilitation service for people who are recovering from illness or injury, or who are disabled, frail or chronically unwell. The aim of CBR is to help people regain their independence and quality of life.
CBR offers an individualised rehabilitation program that is planned in conjunction with the client at an initial assessment visit and then reviewed regularly. This program may include individual or group sessions in the centre, or treatment in the home, depending on need. In most cases, the program will start within one week of the initial visit and will last for a defined period of time.

Community Integration and Leisure Co-ordinator
Dieticians
Medical consultants in Aged Care and Rehabilitation Medicine
Neuropsychologist
Nurses
Occupational Therapists
Physiotherapists
Podiatrists
Social Workers
Speech Pathologists.
CBR also offers Hydrotherapy (water based physiotherapy) at the Sunshine site.
There is no charge for service provided by CBR, however there is a charge for subsidised transport or for staff travel to the home. This service is limited and in most cases clients will need to organise their own transport to CBR.
