Break Thru's Pathways Program focuses on those with a mental illness, who are experiencing, or are at risk of experiencing, primary and secondary homelessness. Working with the client, the main aim is to create a pathway into more stable and appropriate accommodation, as well as establishing support to sustain housing.
The program has a flexible service response to cater for the various needs of the person, as identified by the individual, some of which may include: assistance to find and maintain stable and appropriate accommodation; linking to and liaising with medical and allied health services; support with legal matters and obtaining legal services; practical assistance with daily living tasks; assistance to obtain basic life necessities; and assistance and support to access recreation, educational and leisure options.
The program is designed to assist people who are at risk of homelessness at the point of discharge from an acute care facility within our catchment area and as such this program can only accept referrals from Mid West Area Mental Health Service clinical bed-based units.
