Community Aged Care Packages (CACPs) are individually tailored packages of care coordinated by a Case Manager provided to people 65 years or over with more complex needs. A broad range of services can be provided within a package that is personalised and flexible in order to meet individual goals and needs as they change over time. In order to receive this service a person must be assessed as eligible by an Aged Care Assessment Service (ACAS). Fees apply for this service and are set according to income.
