Additional information
The program provides individually tailored packages of care (Linkages packages) coordinated by a Case Manager to people of any age with complex needs as a 'top-up' to basic Home and Community Care (HACC) services. To be eligible a person must have needs that cannot be met by the mainstream service system and/or be at risk of premature entry to residential care. A broad range of services can be provided within a package that is personalised and flexible in order to meet individual goals and needs as they change over time. Fees apply for this service and are set according to income.
