The Aged Care Assessment Team is a multidisciplinary team whom provide holistic assessment of the needs of the older person in order to promote, maintain and improve their health and well-being. Services will be provided to aged clients living within the Collinsville, Bowen, Proserpine areas. ACAT aim to maximise client independence in the community, prevent unnecessary early entry into aged care, enhance quality of life for clients and their carers, and enable access to Australian Government subsidised aged care services.
