Disability Support Services.
The St. Luke's Disability Support Service is funded by the Department of Human Services under the Disability Services Act (2006) to provide supports to individuals with an identified disability - intellectual, sensory, physical, acquired brain injury. The Program operates in Macedon Ranges Shire and Bendigo.
There are three components to the Service:
Outreach - to assist people living independently in the community. This service provides intensive supports for a small number of people sharing accommodation.
Individual client packages to assist individuals with complex needs to live independently in the community. The Service is flexible and aims to meet individual client needs.
Direct client support may include:
Living skills, Budgeting and banking, Domestic routines,Tenancy issues,Seeking accommodation,Facilitating social access,Linking people to other services, Liaison with other service providers
