WIMMERA PRIMARY CARE PARTNERSHIP:
Facilitates, advises, markets and supports the Wimmera PCP to maintain a Strategic Direction based on the Wimmera Community Health Plan.
This will be achieved by:
-- Creating improved communication between Primary Care Services.
-- Development of effective links through PCP projects between general practitioners, other private providers and the funded primary care services.
-- Supervision of PCP staff or consultants engaged to further the project aims of the Wimmera PCP.
-- Coordination of the development of identified PCP Programs, i.e. better access to services, information provision, quality improvement, health promotion and disease management.
