The Home and Community Care (HACC) program assists residents to maintain their quality of life and well being, and to remain living in their home for as long as possible. Eligibility and priority for services are determined by a phone assessment. An in-home assessment may also be required. Clients new to the HACC program may be required to go through Access2HomeCare.
