The Outreach team supports people living with HIV (PLWHIV) to improve their independence, quality of life and wellbeing. We provide case management for people living with HIV (PLWHIV) and complex needs.
We work in a person centred, strength based and trauma informed way to assist clients to be self-managers in coordinating and managing their desired goals.
