OT service includes:
• Assessment and treatment to maximise independence
• Prescription of assistive equipment eg bath seats, wheelchairs etc
• Minor and major home modifications
• Adapting activities of daily living
• Health promotion and education
Referral requirements:
• Ensure client is set up for safe home discharge
• Ensure you to contact HBAH OT for handover of information for outstanding SWEP
