The Community Support Scheme (CSS) connects individuals with Support Workers who actively work with them to meet their goals and respond to their changing needs. Consumers are engaged in goal setting and have an individual support plan in place which is reviewed regularly. UCWB provides this service in partnership with SA Health and supports individuals with lived experience of mental illness and physical or other disability. CSS works within a recovery model framework to support Consumers to remain connected to the community while maintaining and promoting independence. Support can cover the following areas:
• Social Support:
– Assisting with identifying and linking into activities in the community, building self-esteem and working through social barriers.
• Domestic Assistance:
– Working with the Consumer to build skills and capacity to maintain the home environment.
• Home Maintenance:
- Identifying and assisting the Consumer to address home maintenance issues including gardening and building skills in this area.
To be eligible for CSS a Consumer must be under Care Coordination within their regional Community Mental Health Centre.
