Aged Care Home Packages provides a co-ordinated package of services tailored to meet the clients' specific care needs. A package can include help to stay at home including personal services, support services and clinical care. We give our clients choice and flexibility in the way that care and support is provided to them. Eligible clients must have been assessed by an Aged Care Assessment Team and a level of care has been determined for them.
