Hospital @ Home is a dedicated nursing service based at Flinders Medical Centre (FMC) which provides acute nursing care and support to patients in their own home. The Hospital @ Home nurses provide care for people on their return home from hospital after an illness. This care is a substitute for treatment at the hospital, meaning people can return home sooner or avoid hospitalisation altogether.: EntryProcedures Referrals will be accepted between 8.00am to 9.30pm.
