Home and Community Care (HACC) Domiciliary Services provide a home-based occupational therapy service and home-based physiotherapy service for HACC clients who are unable to attend outpatient therapy.
Occupational Therapy service may include:
• Assessment of client's ability to manage activities of daily living that impact on safely living at home (ie. Bathing, toileting, transfers, kitchen tasks).
• Prescription of complex aids and equipment (ie, shower chairs, wheelchairs)
• Home assessment to determine the need for home modifications.
Physiotherapy service may include:
• Assessment, advice and education for clients and carers
• Assessment of mobility issues around the home or after a fall.
• Short term intervention and goal specific treatment
• Advice and prescription of exercises and/or gait aids
• Referral to other servicesDomiciliary Services provides a home-based occupational therapy service and home-based physiotherapy service for HACC clients (frail, aged and younger people with disabilities and their carers) who are unable to attend as an outpatient.
Fees
No fee for consultation. Costs may apply for equipment.
