The Community Health program provides primary care services that improve the physical, mental and social well being of clients and aims to reduce the requirement for hospital and other specialist services.
All clients in the region are eligible to access community health services however services are prioritized towards population groups and individual who have chronic and complex health issues and who are in lower socio-economic heath groups Integrated health promotion is also a key objective of the community health program.
The service is managed by the Allied Health Ambulatory Care Coordinator and is staffed by occupational therapists, physiotherapists, social workers, speech pathologists, allied health assistants, podiatrists, psychologists (including neuropsychology), exercise therapists and dietitians.
Clients may be treated on an individual basis or in groups. Some therapy programs have been devised for specific groups.
Fees
Fees vary according to the program/group and will be discussed with the client prior to service commencement.
