Rehabilitation in the Home program is designed to enable appropriate clients, with carer support, to undertake all or part of their rehabilitation program in their own home.
Rehabilitation in the Home offers a comprehensive rehabilitation program, coordinated by a case manager, and using a range of therapists according to identified treatment needs. Staff include medical, nursing and allied health disciplines.
The program aims to ensure the continuity of care and facilitate reintegration into the community after a period of hospitalisation.

Eligibility
The program is available to people living within 20 km of Ballarat who are assessed as requiring active rehabilitation after and illness, injury, surgery or deterioration in function. Generally clients will be adults.
In certain circumstances it may be possible to offer services to people in other parts of the Grampians region, subject to availability of therapists.
