Group outpatient rehabilitation programs are an important part of the care and support of cardiac patients and their families.
Based on light exercise and education groups, it aims to improve fitness and to provide support and information, particularly during the early period after an acute illness or operation.
The program is held 10.00 am - 12.00 am, Thursdays at the Community Centre, East Grampians health Service over a 6 week period.

Eligibility:
A referral from General Practitioner is required. The program is available to anyone who has had a heart attack or operation, angina, other heart or blood vessels problem, diabetes, high blood pressure or heart failure.
Aims:
The program aims to; help people return to a healthy and productive life after a heart illness for operation, to help prevent further illness or disability, to improve the health of those people with risk factors who would benefit from an education and exercise program, to help reduce the risk of heart disease in other family members, to give support and advice to spouses and carers and to provide information and education prior to procedures.
It can also assist those who wish to lose weight, stop smoking and be more physically active.
