The Rural Allied Health Team (RAHT) is a multidisciplinary service including the disciplines of Nutrition & Dietetics, Occupational Therapy, Physiotherapy, Podiatry, Social Work & Speech Pathology. The team provides services at home or in the community to frail older people, people with a disability and their carers. This includes education, treatment, therapy and self care strategies to help manage a range of health conditions and meet individual needs. It's about you - working with you to stay active, connected with your local community and stay as independent as possible in your own home. Dietetics: Provides nutrition advice and support to help maximise health, well-being and independence at home and in the community. The team's Dietitians are able to help with many nutritional issues such as unintentional weight loss, underweight and frailty, wound healing, bowel problems, nutritional deficiencies such as iron, osteoporosis, food intolerances, diabetes, home enteral nutrition (HEN), neurological disorders and other conditions/diseases that affect an individual's nutritional status. The emphasis is on person-centred care and capacity building to improve functional independence.
