Additional Information: Clients following a heart attack or heart surgery are assessed and are either seen on an individual basis or join the 8 week cardiac rehabilitation program. The program is run by a team of health professionals including a physiotherapist, nurse, social worker, occupational therapist and dietitian. The aim is to understand heart disease, the causes and how to improve heart health using strategies such as exercise, diet, stress management and relaxation. It also aims to support clients returning to pre-injury activities including work. The program is twice a week (Tuesdays and Fridays, 10.30am to 12.30pm) and consists of exercise and group discussions..

Additional Information: We see persons (adults) with Type 1 and Type 2 diabetes and diabetes in pregnancy. Diabetes Educators available 3 days a week (Monday, Tuesday, Thursday). We run a 5 week self management program for people who are newly diagnosed with Type 2 diabetes/pre-diabetes, or for people who have diabetes who wish to update.

Occupational Therapy services are provided in our HACC (aged and disabled) and multidisciplinary Rehabilitation programs, both at the centre and in people's homes.
Occupational Therapists are able to visit older adults in their homes, to provide assessment, advice and assistance to improve their safety and independence.
This often involves advice on the types of home modifications eg ramps, and equipment that a person may need.
This valuable service can help people remain in their homes as long as possible.
The service can also assist those who wish to have greater mobility and access to community activities to do so. An example of this is assessment for and training in the use of a scooter.
The Occupational Therapist liaises with other agencies to facilitate access and participation in activities and access to aids and equipment where appropriate. tt

Additional Information: MY HEALTH, MY LIFE! is a our week chronic disease self management group program, followed by individual health coaching sessions conducted over a twenty-two week period. An initial health coaching session is conducted face to face, with subsequent sessions conducted over the telephone. The program is suitable for people with chronic conditions and their carers.

Additional Information: Community Rehabilitation provides an outpatient rehabilitation service to people who have experienced an illness, injury or disabling condition. This service aims to help people live as independently as possible. Includes Cardiac and Pulmonary rehabilitation, Upper Limb and Hand Therapy, No Falls program and Exercise Groups. Also provides individual therapy and counselling. The service is available Monday - Friday 8.30am - 5.00pm. A nominal capped fee applies.
