- Identification of client priorities and goals relating to the wound.
- Medical, social and functional assessment.
- General, regional and local examination.
- Wound assessment including digital photography.
- Investigations including sensory and vascular tests.
- Formulation of a diagnosis.
- Collaborative care planning involving entire team and client.
- Utilisation of state of the art wound product technology.
- Communication with other care providers, such as the client's GP, RDNS, etc.

A community based program providing inter disciplinary assessment and management within the clients own home or residential facility. The Aged Care Shared Care team work within the Moreland-Hume catchment area.
Therapists include: Physiotherapist, Nurses, Occupational Therapist, Dietician, Podiatrist, Social Work and Speech Pathologist.
A consultant Geriatrician is available for review of all patients.
Residential component of Aged Care Shared Care:
Support for residents of Aged Care facilities is available if it is thought that benefit would be gained from a management plan / assessment for falls, dietetic, speech or Geriatrician review.
Also available is education sessions, including diabetes, continence and wound management.

BHS Palliative Care consists of a 20 bed inpatient unit, providing multi-disciplinary care to patients with a life-limiting illness, and for whom the primary focus of care is maximising comfort. Patients are admitted for symptom management, terminal care, convalescence and occasionally for family respite.

Facilities comprise two operating theatres and one endoscopy suite
Staff include Nurses, Drs, Theatre Technicians, Clinical Assistants, Clerical, cleaning.
Patients are able to undergo small day procedures with minimal waiting time following arrival at the day centre.
Patients will have had an appropriate pre-operative work up completed prior to attendance at the day procedures area for day surgery.
Access and Referrals
Surgical services via Northern Hospital, Royal Melbourne Hospital and Royal Childrens Hospital have clinical programs at this service.
Referral from local GPs to in-house medical specialists.
