Nutritious 2 course meals are provided each week to older residents and people with disabilities who cannot prepare their own meals. Frozen meals are also available. Meals are delivered from three locations in the Shire - Bannockburn, Rokewood and Linton.

Council engages residents and local commuities in planning for their future. The program assists community members to work together to identify needs, issues and opportunities; decide on priority actions or projects and to establish groups they will work with to achieve agreed goals.
Towns in Golden Plains Shire with the assistance of a community development facilitator have developed their own community plans including: Batesford; Berringa/Staffordshire Reef; Cape Clear/Illabrook/Rokewood Junction; Dereel; Enfield; Grenville/Garibaldi; Haddon; Inverleigh; Lethbridge; Linton; Meredith; Napoleons; Rokewood/Corindhap; Ross Creek; Rural - Wallinduc Area; Scarsdale; Shelford; Smythesdale; and Teesdale.
Council also supports its local communities through it Community Grants Program. The program is available to community organisations for projects that address local needs and provide opportunities that enhance the wellbeing and quality of life of residents.
Grants are allocated each year with application forms available in Feabaruary and funding allocations announced in July.

Maternal and Child Health nurses provide an individual health advice and support service for parents of children up to six years. This service aims to improve outcomes for children and their families. Nurses conduct assessments at key developmental stages, and other services as required.
New Parents Groups are conducted from time to time, according to demand.
An outreach service provides intensive support for vulnerable families who may require additional support.
The Maternal and Child Health service is conducted from various locations across the shire and telephone assistance is available 24 hours a day by phoning 13 22 29.

The Golden Connections community transport service provides assistance to isolated residents who do not have access to transport due to their location, frailty or disability.
The service includes:
-- A Community Car providing a door to door service for eligible clients who need to access medical and other appointments.
-- Dial-a-Ride - a booked mini bus service that operates on set runs, taking people to major towns and facilities.
-- A Community Bus transport for clients to major community activities, events and facilities.Trained volunteers who have passed police and other checks drive these vehicles.

A Living at Home Assessment is a person-centred assessment of the person and their carer's needs, which leads to a care plan and individualised service responses. The assessment and care planning process sits within the service coordination framework and takes an ASM approach to:
• building on the person's and carer's autonomy, by offering choices and focusing on individual strengths and interests
• encouraging & supporting people to regain skills and capacities wherever possible
• strengthening social, community and family connections
A Living at Home Assessment is a broad, holistic needs based assessment and can include: a service specific assessment and care plan for the range of HACC services delivered by the assessing organisation; a referral action plan to both HACC and non-HACC services that cannot be met by the assessing organisation.
A Living at Home Assessment focuses on:
• Social, functional and emotional needs
• Carer and family needs
• Environmental issues
• Strengths and capacities, not just deficits
• Involvement of multidisciplinary expertise
A Living at Home Assessment is a funded HACC activity delivered by designated HACC Assessment Services.
Referral
Referrals are welcomed from clients/carer/family and other service providers.
A SCTT referral is required from service providers – the referral should contain the following:
• Confidential referral cover sheet – clearly outlining the reason for referral
• Consumer information profile
• Summary and referral information profile
• Consumer consent to share information profile
• A copy of the Grampians Region Service Provider Home Safety Checklist (or your equivalent agency safety checklist)
