Additional Information: Aged Care Assessment Services (ACAS) are independent teams who assess the care needs of frail older people aged 70 years and over or 50 years and over for Aboriginal and Torres Strait Islander people. Assessment teams are multi-disciplinary and can include health professionals such as medical officers, social workers, nurses, occupational therapists and physiotherapists. They conduct comprehensive assessments for those needing community services or aged care residential services. They assess for all restorative and potential care options. For an older person to access Commonwealth funded residential care, residential respite, Community Aged Care Packages (CACPs), Extended Aged Care in Home (EACH) Extended Care in Home Dementia (EACD D) or Consumer Directed Care (CDC) packages or flexible care – Transition Care program, they must first be assessed as eligible by an ACAS.

The Community Inclusion Program is funded to provide services for people with an intellectual disability. The program is based in Wangaratta but can provide a service to people living anywhere in Hume and neighbouring regions.
Community Inclusion Brokerage Service (Day Programs funding) provides experiences & opportunities for people with an intellectual disability. The activities are provided through a brokerage model with community based organisations, services & businesses at community based venues. Activities may include drama, art, jewellery making, swimming.
An 'out of hours' social club operates monthly to give informal opportunities for people to make friends while enjoying social activities, eg horse riding, fishing, dancing and cinema.
Futures for Young Adults supports young people with an intellectual disability who have passed through the school system to move onto community based adult options. Examples are TAFE, The Centre for Adult Education, computer course, literacy & numeracy tutoring, driver preparation, work experience, travel training.
